Pathways Academy Senior/Graduate 
 Transcript Request 

This form should be used by:
1.) Pathways’ Seniors who are in need of an unofficial transcript for scholarship purposes.
2.) Official transcript request for a Pathways’ Graduate who did not walk in the official grad ceremony 

Please fill out the online form below and submit it to the Pathways’ school office.  If you to print the hard copy form,  it may be mailed, brought in to the office during Thursday office hours from Noon-4pm (when the office is open) OR dropped into the Communications INbox on the back porch of the Pathways’ Resource Center. 

Transcript Requests.*  require 10 days notice during the school year (September-May) and 30 days notice during the summer (June-August). If the request needs to be expedited, please email the High School Administrator. 

*NOTE: There is a $20 fee for expediting a transcript. This charge includes the fee for certified mailing from the Gadsden CIty post office..SENIOR TRANSCRIPT
 ONLINE TRANSCRIPT FORM 

Pathways’ Alumni Transcript Request 

This form should be used by former Pathways’ graduates who need to  request an official transcript be sent to their college or an employer.  Please print the form and submit it to the Pathways’ school office.  Forms may be mailed, brought in to the office during Thursday office hours from Noon-4pm, or dropped into the Communications INbox on the back porch of the Pathways’ Resource Center. If the request needs to be expedited, please email the High School Administrator.

ONLINE TRANSCRIPT FORM

Notice of
Withdrawal Form

Any family choosing to withdraw from Pathways Academy is required to submit a notice of withdrawal for the child(ren) who are withdrawing. 

Please print the form and submit it to the Pathways’ office.  Forms may be mailed, brought in to the office during Thursday office hours from Noon-4pm, or dropped into the Communications INbox on the back porch of the Pathways’ Resource Center.

If an official transcript is needed, please print, fill out, and submit the necessary transcript request form* along with your withdrawal notification to the Pathways’ office.
Current Pathways’ families may print an unoffical transcript from their Homeschool Reporting Account to submit to the requesting school.

WITHDRAWAL FORM

Transcript Requests:
K-11th Grade Students

This form should be used by families who are withdrawing a student and need an unofficial transcript to enroll in the new school. This form should also be used for any current student in grades K-11th in need of an unofficial transcript for insurance, student verification, scholarship purposes, etc. Please print the form and submit it to the Pathways’ school office.  Forms may be mailed, brought in to the office during Thursday office hours from Noon-4pm, or dropped into the Communications INbox on the back porch of the Pathways’ Resource Center.* 

Transcript Requests require 10 days notice during the school year (September-May) and 30 days notice during the summer (June-August). If the request needs to be expedited, please be sure to contact Pathways Academy.*
